Pregnancy math can be a little murky. After all, unless you went the IVF route, your physician has no way of knowing the exact time of conception. Because of this, pregnancy is calculated from the first day of your last menstrual period or LMP. From that day, weeks of pregnancy is officially counted, and your doc or midwife busts out the red pen and marks your due date on the calendar. Sometimes, however, the due date may be revised. It can be pretty difficult to determine a due date using the date-of-last-period method for women with irregular cycles — which is why an ultrasound exam is often required to determine gestational age. An early ultrasound, usually done at about 6 to 9 weeks, can give the most accurate due date estimation by measuring the size of the embryo or fetus.
Prenatal tests are tests done during pregnancy to check a woman’s health and her baby’s. They can detect conditions that can put a baby at risk for problems like preterm birth if they’re not treated. Tests also can help health care providers find things like a birth defect or a chromosomal abnormality. Some prenatal tests are screening tests that can only reveal the possibility of a problem. Other prenatal tests are diagnostic tests that can accurately find whether a fetus has a specific problem.
A screening test sometimes is followed by a diagnostic test.
First trimester growth restriction may predict miscarriage or adverse outcome later in the pregnancy, but determinants of early growth are not well described. Our objective was to examine factors influencing fetal and gestational sac size in the first trimester. Prospective observational study of singleton pregnancies before 12 weeks gestation. Maternal characteristics ethnicity, maternal age, obstetric history, abdominal pain and vaginal bleeding , crown rump length CRL and mean gestational sac diameter MSD were recorded.
A stepwise linear mixed effects analysis was performed to determine factors influencing rate of change in CRL and MSD. Maternal age also influenced MSD: older women had gestational sacs which were 0. Rate of increase in CRL was greater in fetuses of black versus white women and increased with advancing maternal age. As CRL is used to date pregnancies, and this influences further growth assessment, consideration should be given to the use of individualized growth charts which take account of maternal factors found to influence first trimester growth.
Early pregnancy dating by crown rump length CRL , rather than estimation of gestation from last menstrual period LMP dates, is commonly performed to date pregnancies up to 14 weeks NICE recommendation, Dating at this stage forms the basis for decisions regarding timing of delivery and assessment of growth during the second and third trimesters Mongelli and Gardosi, ; Gardosi, Pregnancy dating by transabdominal sonographic measurement of CRL was described by Robinson and subsequent studies have shown similar growth patterns using high-frequency transvaginal sonography Hadlock et al.
These studies assumed uniform embryonic growth regardless of maternal characteristics.

Expected day of delivery from ultrasound dating versus last menstrual period—obstetric outcome when dates mismatch. John M. When the expected date of delivery was postponed more than 7 days as a result of ultrasonographic dating, the risk of a small-for-gestational-age infant increased. Conventional perinatal wisdom, since the inception of obstetric ultrasonography US , has been that disordered growth in utero occurs only in the second half of pregnancy; growth in the first half of pregnancy is believed to be uniform, with little variation among individuals.
This assumption of uniform growth at the beginning of gestation allows us to create growth curves for populations and generate estimates of gestational age for individual fetuses from their growth parameters.

The yellow bars show the gestational age when different organ systems are sensitive to functional defects and minor malformations. For example, thalidomide causes limb defects when taken during the the 21st to the 36th day after conception. Tetracyclines do not produce their effects until after the first trimester when the skeletal system begins to form bone. Some drugs such as ethanol may produce harmful effects on functional development throughout pregnancy.

A dating scan is an ultrasound scan to determine how many weeks pregnant you are and your due date. Read on for more information about what a dating scan offers. A dating scan might be recommended before 12 weeks to confirm your due date if you are unsure of your last menstrual period or your date of conception. The scan is optional and not everyone will have it.
If you choose to have a dating scan, it will most probably be your first scan. In the second trimester of pregnancy, you may be offered to have an morphology.
A first trimester early pregnancy and dating ultrasound is an examination you have during the first 13 weeks of pregnancy. Due to the size of the developing structures and embryo, it is uncommon to perform an ultrasound before 5 weeks gestation 5 weeks after the start of your last menstrual period. Ultrasound is not consistently able to confirm viability and fetal heart activity prior to 6 weeks gestation, so unless there is an urgent need to have an early pregnancy and dating scan earlier most of these ultrasounds are performed after 6 weeks and ideally at 8 weeks for dating purposes.
Urgent cases may still be performed earlier, please discuss this with your referring doctor. Your ultrasound may be performed transabdominally or transvaginally. Transvaginal ultrasound involves placing a thin transducer slightly thicker than a tampon into the vagina. The transvaginal ultrasound can offer extra detail to the examination as it allows the ultrasound probe to come into close proximity to the uterus womb. A transvaginal scan is optional for all patients and it is your decision whether to proceed with that part of the examination.
Ultrasound, both transabdominal and transvaginal, is considered to be safe during the first trimester of pregnancy.

Your anomaly scan also known as your 20 week scan. At your 1st trimester scan, an appointment will be made for your anomaly scan.
During this scan, the sonographer will have a detailed look at your baby. This is part of the fetal anomaly screening programme FASP. For more information about the anomaly screening programme click here. Please note this scan is not a sexing scan. Criteria for these scans varies but includes having a previously small for dates baby, being a smoker or having a BMI above These scans are usually held from 28 weeks, and every weeks thereafter to keep an eye on the growth of your baby until delivery.

BMUS published their first Working Party Report on fetal measurements in , at a time when the practice of obstetric ultrasound remained varied, with obstetric units having quite widely differing protocols for the number and timing of scans offered, as well as policies on re-dating pregnancy from ultrasound measurements.
That report offered recommendations for the use of validated published tables and formulae for the commonly acquired fetal measurements used in dating and monitoring fetal growth. Since then, practice across England and Wales has become more uniform, particularly following the publication of the NICE guidance on antenatal care 1.
BMUS accepted the need to review the old guidance, in order to ensure that the statistical validity of the original recommendations remained intact. That review, performed by Dr Lynn Chitty, Dr Trish Chudleigh and Dr Tony Evans, did bring some changes to recommendations, most particularly that dating after 13 weeks be based on head circumference measurement rather than bi-parietal diameter.
The revised guidance was published in February , and was widely welcomed. However, it was not long before problems arose with the crown rump length formula. Whilst the formula recommended was in keeping with that used in many obstetric ultrasound units, it quickly became apparent that the BMUS formula was not the same as that used by the Fetal Medicine Foundation in their first trimester programme. There followed a considerable amount of work by a relatively small group of dedicated souls, chaired and led by Pat Ward of the National Screening Committee which identified the lack of a perfect formula for calculating gestational age from crown rump length.
This should remove the confusion that has been apparent over the last year or so, and ensure uniformity of dating for screening purposes.
